The case study features best practices exemplified by the Community Health Center of Southeast Kansas (CHC/SEK) and shows how they integrate community health workers into delivery of the Medicare Chronic Care Management benefit and billed CHWs' time to Medicare.

"Community Health Workers are a value add to care management teams. CHWs offer expertise in resource navigation, care coordination and support that patients living with multiple chronic conditions often need to achieve their health goals and care plans."

Leah Gagnon, Senior Director of Patient Engagement and Wesley House, Community Health Center of Southeast Kansas

The brief provides:

  • An overview of Chronic Care Management as a Medicare primary care benefit and service elements.
  • How CHWs are engaged as an integrated part of an organization’s healthcare team.
  • The benefits to the health care organization and to the patients through CHW services.
  • Outcome measurement.

This brief is a follow up to the Reimbursement Strategies for Employers of Community Health Workers toolkit, which provides a blueprint of how health care agencies can find funding for CHWs.

The report was prepared by Erika Saleski, ES Advisors LLC, with assistance of the Community Health Center of Southeast Kansas, and financial support from the Health Forward Foundation and the Kansas Department of Health and Environment.
