Case Study of Chronic Care Management released

The KC Regional Community Health Worker Collaborative recently released a report, Community Health Worker Sustainability – A Case Study of Chronic Care Management, that uses a real-world example of how health care teams can incorporate Community Health Workers (CHWs) into their operations. MARC provides support to the Collaborative.

The case study features best practices exemplified by the Community Health Center of Southeast Kansas (CHC/SEK) and shows how they integrate community health workers into delivery of the Medicare Chronic Care Management benefit and billed CHWs' time to Medicare.

The brief provides:

  • An overview of Chronic Care Management as a Medicare primary care benefit and service elements.
  • How CHWs are engaged as an integrated part of an organization’s healthcare team.
  • The benefits to the health care organization and to the patients through CHW services.
  • Outcome measurement.

This brief is a follow up to the Reimbursement Strategies for Employers of Community Health Workers toolkit, which provides a blueprint of how health care agencies can find funding for CHWs.
